Our Commitment to Families
At The Unique Scholars Institute, we believe in building strong partnerships with families. Parent and guardian involvement is essential to each student's success. We require documented consent for all program participation to ensure transparency and trust.
Consent Center
Consent forms for our programs are completed with our staff during enrollment and registration. Online signing is not open yet — when a consent form is approved for online signature it will appear here with its version and effective date, and you will be able to review and sign it from your family account.

Enrollment Consent
Before a student can participate in any of our programs, we require:
- Completed enrollment application with parent/guardian signature
- Emergency contact information and medical disclosure
- Acknowledgment of program rules and expectations
- Agreement to pickup/drop-off procedures
- Authorization for emergency medical treatment if needed
Photo/Video Release
With your permission, we may photograph or video record students during program activities for the following purposes:
- • Internal progress documentation and parent communications
- • Program marketing materials (website, social media, print)
- • Educational showcases and presentations
- • Fundraising and donor communications
Photo/video release is optional. Parents may opt out without affecting program participation.
Liability Waiver
Participation in physical activities (Panther Performance Pass training, field trips, etc.) requires a signed liability waiver acknowledging the inherent risks of physical activity. While we maintain comprehensive safety protocols and insurance, we ask parents to acknowledge these risks as part of the enrollment process.
Authorized Pickup
For the safety of all students, we require:
- • A list of authorized individuals who may pick up your child
- • Photo ID verification at every pickup
- • Advance written notice for any changes to authorized pickup persons
- • Emergency contact notification if pickup is delayed
